Insight into Human Error
(1 day)
This one-day program introduces your organization to concepts and practices that can systematically and proactively identify root causes of human error as well as address them. It begins with an experienced Oriel consultant conducting a "walkabout" to assess existing vulnerabilities in your organization. Next, the first of two presentations explains why even experienced and committed people in well-run organizations can be vulnerable to human error. The second presentation introduces methodologies to identify risk-influencing factors (RIFs) and offers ways of avoiding or at least reducing the effects of RIFs. Finally, based on the initial walkabout, the consultant discusses any evident potential for error and ways in which it could be addressed.

Confronting Human Error
(5 Days)
This five-day package of training and consulting provides your organization with a practical demonstration of how human error can be addressed proactively and systematically. It begins with two days of classroom-based explanation of causes of error and the process for identifying vulnerabilities in an organization. In the two days that follow, the Oriel consultant applies the methodology to identifying the potential for error in the organization, with a practical exploration of its operational processes. This provides an opportunity for a small group to see for themselves how the error risk reduction process identifies risk-influencing factors. The final day is spent explaining the findings and considering options for addressing them.

Error Risk Reduction Core Program
(7 Day)
This dynamic course is designed to develop the organizational competence to systematically and proactively identify and address contributory causes of human error. To begin transformation from pursuing local causes of error to enhancement of human reliability across the organization (or a defined part).
Whichever option you select, our experienced ERR specialists will work with you on-site to conduct a variety of ERR activities, such as:
- Investigate serious incidents and provide guidance to avoid repetition.
- Assess vulnerability to error in specific activities or broader systems.
- Provide guidance on building an organization with enhanced human reliability.
- Examine “failure critical” tasks in detail to optimize human reliability.
- Review plans for new or changed activities or facilities to assess error risk as well as plans for one-time events (e.g., conferences, launches, etc.).
- Advise on error risks in organizational expansion, mergers, downsizing, etc.
- Provide advice to professionals in other disciplines (PR, HR, legal, etc.).

Implementing a Human Error Risk Reduction Program
(12 Days)
Oriel recommends that organizations develop a core team to lead and support risk-reduction activities. To help accomplish this, Oriel can provide the training needed to establish and enhance the core team’s capabilities. This consists of some classroom activities and a lot of supervised live project work to begin the practical process of risk reduction. Although initial training can take less than two weeks, this rapid buildup of know-how enables the core team to broaden the scope of its activities to become a valuable corporate asset. Within six months, the team may be able to transition from merely addressing local and generic risk-influencing factors (RIFs) to beginning the strategic transformation needed to increase human reliability more widely across the organization.
A core team of up to 16 people can build the capability to:
- Proactively identify and address root causes of vulnerability
- Investigate serious incidents and recommend ways of avoiding reoccurrences
- Build awareness throughout the organization of why errors happen and how to avoid them
- Develop a corporate knowledge base about error and deploy it widely
- Operate a risk reporting system
- Support initiatives to address RIFs
- Provide advice to those planning new projects or changes
- Maintain an overview of the potential for error across all organizational activities
Error risk reduction (ERR) is a practical approach to addressing real-world problems. Although a foundation of understanding can be laid in the classroom, working on "live" exercises creates conditions in which team members can learn the methodologies most effectively. This 12-day program provides enough training and support to gain a good working knowledge of the principles and practice of ERR.
